CSB releases video animation of fatal chemical release at DuPont's La Porte facility

The US Chemical Safety Board has released a video animation of the November 2014 accident at DuPont’s La Porte facility, located east of Houston, which killed four workers and injured a fifth when methyl mercaptan, a toxic chemical used in the company’s insecticide and fungicide manufacturing process, was released.
After the initial investigation into the four deaths, the US Department of Labor's Occupational Safety and Health Administration (OSHA) found hazards that prompted the inspection at the facility to be expanded under the National Emphasis Program for chemical facilities.
