Fatal Indian steel plant explosion report exonerates VSP management (21 August 2012)
The three-member high-level committee which inquired into the blast at the Visakhapatnam Steel Plant (VSP) in the southern Indian state of Andhra Pradesh that killed 19 employees in June has said that site management were not responsible for the blast, according to the Times of India. Based on statements the panel gathered from surviving officials, the committee concluded that the fire might have occurred in the outlet passage of the oxygen system filter.

Explosion kills 16 at Indian steel plant (14 June 2012)
An explosion on the night of June 13 at the Viskhapatnam Steel Plant (VSP) in the southern Indian state of Andhra Pradesh killed 16 workers and injured a further 20, according to the Times of India. The incident, the worst-ever in the history of the accident-prone plant, was the result of a massive blast at the recently-commissioned oxygen plant in the steel melting shop (SMS) III unit.

Two killed in India steel plant incident (04 May 2012)
Two workers were killed in an accident at a new blast furnace at the Visakhapatnam Steel Plant in Andhra Pradesh on May 1. The two, engaged by a private company, were burnt alive following an explosion in the blast furnace.
