    The US Chemical Safety and Hazard Investigation Board (CSB) has released a new safety video on its investigation into the fatal release of acetic acid at a LyondellBasell plant in Texas on 27 July 2021 which killed two contractors. Approximately 74 tonnes of the highly corrosive acid mixture erupted from equipment when workers inadvertently removed critical components of a plug valve. Three contractors were sprayed with the acid, killing two of them....

    Federal investigators have cited a US tank cleaning company after it failed to ensure that atmospheric testing was done inside a tank before a 53-year-old employee entered and suffered fatal injuries. New research says 94% think that UK Health & Safety legislation needs ‘overhauling’ 10 July 2024

New research has found that more than nine in ten workers believe that the main piece of UK Health & Safety legislation should be overhauled, fifty years after it was first introduced. The figure calling for an overhaul rises to 97% amongst managerial staff, with the need for a ‘greater focus on mental health and wellbeing’ seen as the most important factor (82%) in a future rethink of health and safety, and also a key reason in 2024 that people feel safer in the workplace (50%)....

Explosives manufacturer fined £670,000 after worker dies 04 July 2024

A manufacturer of military explosives has been fined £670,000 after an employee was killed and another seriously injured in an explosion at a factory near Salisbury, UK. The two men were working on the production of MTV – an explosive substance used in military flares – at the premises of Chemring Countermeasures on 10 August 2018. They were cleaning a vessel used in the production and were removing residual explosive material in preparation for the next day’s shift....
